Overview of Regenerative Injection Therapy
Platelet Rich Plasma (PRP) / high dose PRP / PRF / Prolotherapy are all new cutting-edge options for healing, regeneration and pain relief. The process works by injecting regenerative solutions into the site of pain, injury, or connective tissue damage for healing.
The ultimate goal of regenerative injections is pain relief and increased quality of life for the patient. Platelet based injections act in a number of ways due to the functions of the various growth factors that they store and release. This includes a local immune reaction, clearing of cellular debris, modulating inflammation in the area, cellular recruitment and collagen remodeling. The Orthobiologic solution also has a secondary downstream effect and may activate and recruit native stem cells through protein, growth factor and RNA signaling reactions into the treatment area for connective tissue healing of tendons, ligaments and cartilage. (1)(2)(3)(4)(5)(6)(7) These unique features all have a cumulative impact on connective tissue healing and target the root cause of the patients pain. After the treatment has initiated a regenerative response to the damaged area, many patients notice increased function and pain reduction as structural integrity is improved or restored.

Regenerative Orthobiologic Shoulder Pain: Case Study
Bob Smith (fictional name) is a 88 year old man who presented to my Gilbert, AZ clinic with left shoulder pain for 5 years duration. He had previously been evaluated for the shoulder pain by his primary care doctor who had prescribed a common pain medication to him, Tramadol, but he did not improve from the medication. He eventually was referred to an orthopedic specialist who ordered an MRI of his left shoulder. The MRI revealed tendinitis of one of his main shoulder tendons as well as a partial labral tear and degeneration. After looking at his MRI, the orthopedic specialist recommended shoulder surgery to relieve his pain.
Bob did not believe he would be able to handle the surgery and its recovery rehabilitation due to his age. He began looking for other options available for his shoulder pain and found regenerative injections with PRP and my clinic in Gilbert, AZ. Initially, Bob did not know much about regenerative injection therapy but decided to give the treatment a try in a last attempt at pain relief before surgery.
After taking the case, reviewing the medical history, conducting a musculoskeletal ultrasound examination, and completing orthopedic physical examinations, I noted the following.
- Shoulder Pain (average 5/10, worst 8/10)
- Dramatically decreased range of motion
- Tendinopathy
- Diminished muscle strength
- Impaired daily living activities
- Pain while sleeping
- Constant dull, aching pain
After examining everything and reviewing his MRI report, I decided Bob was a good candidate for regenerative medicine. We began treatment with high dose Platelet Rich Plasma (PRP) injections, due to the patients advanced age, into his shoulder joint as well as the surrounding rotator cuff tissues that support the joint, including his tendons. Bob tolerated the treatment fairly well, with minimal pain during the procedure. I then followed up with Bob after 6 weeks to allow the shoulder to properly heal. At this point in time, Bob reported that his average shoulder pain was now at 0/10 overall and that he had increased range of motion of his shoulder. He had improved so much that he now was able to eat food with his left hand again and drive his car with left hand again as well. I did some follow up evaluations and noted his improvements.
Conclusion
Regenerative Orthobiologic shoulder pain relief is an beneficial option for various shoulder pain conditions and can help patients avoid surgery in some select cases as noted above. Bob had been dealing with shoulder pain for so long that his quality of life was greatly diminishing. Regenerative injections with PRP help the shoulder joint heal and treat the root cause of pain. Cartilage, tendons, and ligaments are strengthened and healed, which reduces pain and increases range of motion. PRP or high dose PRP should be considered as an alternative to shoulder surgery, opioid medications and cortisone injections for certain patients. From my experiences, regenerative injections are quite effective and will surely play a growing role in the evolution of pain management and orthopedic medicine.
